You do not need an appointment. Feel free to walk into any of our locations during our operating hours and be seen in 15 minutes or less.

You must bring a current government issued photo ID, doctor’s test request form, and your current insurance card, if you are wanting to bill you insurance.

Some tests, such as glucose or cholesterol, may require you to fast for at least 8 hours before your sample is collected. Fasting allows for a much more accurate baseline sugar and cholesterol level. When asked to fast, please refrain from eating or drinking anything, except water, until the test is complete.
